Application form for assistance
Documents to be provided:
- Original handwritten application (be sure to indicate that the parents do not object to the placement of information on the stand, on the Internet and in the media).
- A photo of the child (a regular landscape photo).
- Certificate of family composition.
- Certificate of income.
Copies of documents:
- Medical certificates, extracts, doctor's prescriptions.
- A copy of the passport and a copy of the parents' identification code.
- A copy of the child's birth certificate.
- Bank details in IBAN format.
